It includes some personal and potentially sensitive questions, and we are committed to keeping your information confidential. Your application will be reviewed by the Groups Pastor and may also be seen by the Groups Coordinator and Doxa’s elders.

My Testimony of Personal Faith in Jesus Christ

In order to become a member of Doxa Church, you need a clear testimony of personal faith in Jesus Christ. Writing about the time when you trusted Christ as Lord and Savior is a great way to reflect on how God has drawn you to Himself and changed your life. As you write, make sure the focus is on God, what He has done, and is doing in your life. Please follow this outline to write your testimony.

Spend a brief time on “before.” This is not a biography from childhood. It is best to start at a
time related to your experience with Christ.

When and how did you trust Christ as Lord and Savior? *

Be as specific as possible about surrendering your life to Christ. What was happening
at the time? How did Christ get your attention? If possible, share the month and year you
were saved.

Describe one or two ways Christ has changed your life: *

Be practical. Explain how Christ has transformed your life. Share how you are growing in your walk with Christ.

“Just as you have received him, so walk in him, rooted and built up and strengthened in the faith.”
(Colossians 2:6-7).

We desire that most of our groups start by multiplying out of a current group, but for the sake of mission we happy to start brand new groups in new areas.

How many children can you accomodate? *

If your group is able to welcome couples with children, please let us know how many kids you can accommodate. This information helps us create a warm and welcoming environment for families. (If you’re open to families, we recommend starting with one family at a time.)
Examples:

“We’re okay with up to five kids ages 10+.”

What day of the week, time and location are you hoping to have your Small Group meet? *
